My mother spent her final days here, and finding this facility truly felt like a blessing for her and for our whole family. Alex, Connie, and the rest of the staff were wonderful. Alex cared for my mother with such kindness and respect. May God continue to bless the staff and the patients here, and may He give them the patience and resilience to keep moving forward.

DO NOT TRUST THIS FACILITY WITH YOUR LOVED ONE. If your loved one needs professional medical care after a hospital stay, please think very carefully before choosing this facility. While the building looks nice and the rooms are private, that is where the positives end. In our experience, the medical oversight was deeply concerning. The physician refused to adjust treatment plans despite clear and ongoing signs of decline. Even as our loved one showed both objective and obvious symptoms worsening over time, no meaningful changes were made. There appeared to be a lack of proper assessments, and when serious issues were identified — primarily because family members insisted on testing — there was little urgency or corrective action. Staff did not consistently follow orders and did not seem to recognize signs of medical decline. Concerns raised by family were often minimized. For three days we requested that our loved one be transferred to the ER due to significant changes in mental status and severe migraine symptoms. The physician refused. When EMS was finally called, emergency personnel immediately recognized the seriousness of her condition. We had to sign a waiver to transfer her to a hospital 20 minutes away because she was considered unstable — yet staff at the facility continued to say she was “fine.” Within days of transfer, her condition deteriorated to the point where she could not speak or swallow and became nearly unresponsive. We were preparing funeral arrangements. She later described experiencing severe hallucinations and believing she was in hell. It was profoundly traumatic for her and our entire family. We also discovered she was not receiving the correct dosage of a critical medication that had been ordered by her previous facility. This likely contributed significantly to her decline. Had we left her there even one more day, we firmly believe she would not have survived. Our loved one had none of these issues prior to admission to this facility, and since receiving proper hospital care and being restored to her previous treatment plan, these issues have resolved. We will be filing formal complaints with the appropriate medical and licensing boards. In our opinion, this facility is not equipped to safely manage medically complex post-acute patients. If your loved one only needs stable rehabilitation, it may be adequate. But if they require close medical monitoring, I strongly urge you to consider other options. Our family went through an avoidable nightmare. Please advocate aggressively for your loved ones.

DO NOT PUT YOUR LOVED ONE HERE . In the past month they have ran out of medication/ didn’t get ordered. And managed to lose / misplace it . This has not only happened to my family member but several others who are there as well .obviously management is not on top of taking care of these people . I’ve filed a few complaints with the medical board as well now so hopefully no one else will have to deal with this and suffer .

In January 2025 my mother was admitted to Trellis Chino. I have to say that this place is everything it appears to be, five stars all the way! Trevor and his team keep this place running smoothly. The layout is perfect for the person who is taking physical therapy here and trying to heal mentally and physically. The physical therapy team does an amazing job. The nursing staff is on top of it. My mother still asks to go back to this beautiful place.

My mother broke her back and had surgery. She came to Trellis after her surgery and was there 3 months. I have been to other rehabs. This one looks like a nice hotel instead of.a dreary nursing home. They have private rooms and baths. It has a good atmosphere. The staff is helpful and courteous. The rehab was great. Always remember to be with your loved one every day and the staff pays more attention if you are there and on top of things, just like in the hospital!

My relative was admitted to Trellis in May 2024 and discharged in early Oct. unfortunately, a prior facility failed to provide appropriate nursing skills and she was verybdeconditioned on arrival. Great team work and careful slow work, she was successful in gaining back strength to continue therapy at home. Kudos to Emily from Speech Therapy; Roya, Robert, Raymond, Lindsay and Eunice from the therapy department. She is back to walking using a walker and soon be walking independently! Love licensed staff RN’s and LVN’s watched over planning and progress- Jodi, Lila, Ben, Gio, Arranza, Liz, Tracey, Adrianna, Tracey, Eddie and Jose were excellent. Nurse aides Mayra, Daisha, Merlin, Maiacela, Jonathan, Guillermo, Anna were so caring and encouraging. You each made a difference! The Administrative team listened to concerns, worked to make adjustments and changes to correct problems. The facility is clean, well maintained and the EVS staff engaged. Food service is a work in progress. Diets are bland, dry and need some work! Thank you all so very much! I recommend Trellis to all who need help and can work hard! Case manager Yuri was key to communication and a clone! All our best!

After going to another facility for 48 hours I was so thankful and love the staff at Trellis. I think it would be hard to find such a nice, helpful , caring place as this. If I could give it 6 stars I would…

If I could give 0 stars I would, I’m typing this as my loved one is being carried out of here back to the hospital in an ambulance. The fireman himself said “we’re down here all the time this place is horrible you must advocate for your loved one” Genuine patient care is non existent these days! Stay healthy because you don’t want to get ill in this day and age things just aren’t the same. Truly sad. DONT BRING YOUR LOVED ONE HERE TO RECOVER!

My father was transfer here in august 2023 from kindred in Ontario. He has Pancreatic cancer and osteoporosis and malnutrition. So he needed extra physical therapy to gain strength and gain weight to give a better chance with the cancer. Nurses here are amazing and are so kind. Wound care also is absolutely amazing! They know how to provide professional care. The one star is for Administrative and Physical Therapy, my father was admitted and I would go everyday and ask the physical therapist how is he doing. They would always tell me hes doing good and getting there, my father went into there walking and came out completely bedridden. I ask to speak to whoever in charge to explain to me what happened and no one can give answers. Administration/social worker is absolutely horrible! Lady never follows up and I was not notified of his discharge till the day before! Someone called me stating they needed my approval to send him to a long term care because his physical therapy still needed help. I brought him home and thanks to this facility his care was completely gone backwards and no hope of him walking again. DO NOT SEND YOUR FAMILY MEMBERS HERE if they need physical therapy.

The Trellis Chino team has been providing exceptional care for my father postop — at an older age it has been somewhat difficult for him to adjust & their Team has been compassionately understanding and as attentive as possible! The facilities are also great & complement their care 🙂 We are grateFull for all the support of my mother & care of my father

This place is very clean and you do have your own private room and bathroon and shower and good PT dept and the nurses are nice but overworked.. Have only seen a Dr once only after being here 2days. Food is not Diabetic or Chonic Disease Friendly unless you are very vocal about it small portion unless you are vocal about this also. Medication has only been right 1 day. Take long acting insulin in the morning the nurse tried to give me 2 more units after dinner because it was 187 before she checked my chart. Suppose to check my sugar before breakfast lunch and dinner. On Satruday 2/19/2022 had to ask for my meds and to have my sugar checked twice. Didnt get it done till after dinner nurse said she was late because they are short staffed. What do they do about a patient you is older and can’t speak up for themselves. Very concerned about patients who are sicker than me with no family or advocate to speak for themselves Rooms are private very clean but the medication and meals and lack of nurses are a big concern. Arrived 2/11/2022 still having problem after over a week here. UPDATE finaly got an appt for an MRI the ball was dropped no one made the appt for me. Been here almost 2 wks and no one followed thru with this appt. UPDATE #2 went for MRI order was for wrong knee how can one place make mistake after another!!!!! UPDATE #3 After 2 wks still problem asked to be released one prescription filled by them sent it to wrong pharmacy and same brand of insulin but wrong type

Really bad service my uncle was there this past 4 days we had him removed to the hospital
Do not go to this facility. My brother is currently there he is diabetic and just had his leg amputated. He has been there 3 days so far to continue his physical therapy and wound care, but hasn’t been moved yet. He has asked numerous times about his therapy and they all ignore him.. We will be bringing him home soon to care for him.
I would like to give this place higher grade, but cannot due to several issues. Pros: very clean, individual rooms, friendly staff and front desk, good physical therapy Cons: food is not good, they are understaffed, so patient calls and they come 45 minutes later if not longer. Staff changes all the time
I nave been at Trellis for almost 10 weeks during that time four (4) separate medical appointments were missed due to a variety or reasons that include transportation failure to notify patient not to eat food prior to appointment to name a few. I have also returned from medical appointments with doctors orders that were and still are not being fulfilled. This is my life we are talking about…. My recovery so that I am prepared for a major surgery DEPENDS on these doctors orders be followed to the letter and in a timely manner. NIETHER of these are being done. When bringing my concerns to staff by me personally I receive NO clarity. I don’t have family or friends that can challenge this facility to do better….at times I’m TERRIFIED
0/5 stars. Not diabetic friendly, the patient ended up back into the ICU due to lack of insulin and ketoacidosis and forced her to eat yogurt and sweetened foods if she wanted her insulin. She requested many times to come back to the hospital their care was so bad.
I have a friend who is presently there and, to all appearance, is being well cared for! I daresay even “spoiled!” Get well my friend.
Trellis Chino is a God Send and Life Saver! They truly Care about their patients. I was very ill and depressed when I first arrived and they quickly turned my world around. They’re superseding my expectations. They’re doing excellent in helping me heal. Everyday I feel better and better. Thank you too every staff member at Trellis for being personable and professional with much love for Candace, Brenda, Muriel, Veronica, Rebecca, Jet, Julie, Leo, Amy and Ashley.
